Free Initial Consultation

We offer a free initial consultation to anyone that may need home care assistance.  A highly trained care coordinator or nurse will complete your initial consultation and provide education and suggestions regarding your care options.

Apple Tree Home Care does not send sales representatives to meet with potential clients. Our care coordinators are individuals that have a healthcare background and want to make a difference in the lives of seniors. They will spend time to get to know the individual requiring assistance, the family, and the situation. Together with our client’s and families, we come up with a plan. Many times clients and families contact us when they are in a hospital or rehabilitation facility. If this is the case, we prefer to visit the client prior to discharge so we can gather information, talk to healthcare providers, and develop a plan of care before discharge. We have found that pre-discharge planning and coordination is essential to our client’s successful return home.

On the other hand, if you are at home and require assistance, our care coordinators will come to your house and complete the initial consultation. The consultation will include an evaluation of your general health, environment, and needs. The goal of Apple Tree’s initial consultation is to help you come up with the best possible plan for your short and long term care needs.
